There are many factors that affect the appearance of our eyes. Familial or ethnic traits and tendencies account for how our eyes look as a child. As we grow older, the aging process, exposure to sun, gravity, and our own personal expressiveness play a part in how our eyes appear to others. Breast lift surgery re-shapes the breast gland to create a perky, more youthful bust. THE MASTOPEXY PROCEDURE The breast lift procedure involves re-shaping the breast tissue. Dr. Schuster will create incisions to do this. Although this surgery does create visible scars on the breasts, Dr. Schuster wants each of his patients to have results they’ll love. He creates the most minimal incisions and places them where they are least obvious. WHERE ARE INCISIONS MADE? The incisions chosen by Dr. Schuster depend on the degree of laxity and sagging tissue. Circular incisions may be made around the areolas only, or these circular incisions may also extend vertically down below the breast crease. Nipple sensation should not change. The breast lift procedure does not increase breast size. Breast implants can be added to the lift if you want fuller breasts. PREPARING FOR BREAST LIFT SURGERY Dr. Schuster and the staff make sure you have complete information and know how to get ready for surgery. Pre-op guidelines include: Go to a pre-operative physical checkup with your family doctor Stop taking aspirin and products containing aspirin two weeks prior to surgery Inform us if you get sick with a cold or any other illness in the two weeks prior Take a multi-vitamin containing vitamin C, mineral and iron in the weeks prior DO NOT smoke for three weeks both before and after surgery Make arrangements with a spouse, family member or friend to pick you up after your breast lift procedure and stay with you for at least 24 hours Don’t take herbal supplements or supplemental vitamin E in the two weeks prior (small amounts of vitamin E in multi-vitamins are fine) Two days before your breast lift appointment, you’ll fill your prescriptions, buy light foods from the grocery store, purchase Betadine™ soap and any products you want during recovery. Shower using your Betadine™ soap the night before surgery. You should also eat a light meal that evening, get a good night’s rest, and do not eat or drink anything after midnight. Don’t eat or drink anything when you get up in the morning before surgery. We’ll let you know not to wear any jewelry, hairpins or hairpieces. Wear comfy, loose clothing. BREAST LIFT RECOVERY You’ll rest in bed when you get home. It’s important to get up and walk around every hour or so to get your blood flowing; don’t stay in bed. You can also move your legs around while you’re resting to keep blood circulating. Stick to liquids for the rest of the day after surgery. Progress to a light diet as you feel up to it. Discomfort should be alleviated by pain medications, or extra-strength acetaminophen or ibuprofen. Bandages will need to be kept clean and dry. We’ll instruct you how to sponge bathe. Bandages will stay on for about a week. We’ll give you information about buying a post-op bra to provide the right kind of support about a week after surgery. You’ll continue to wear it for about a month. If you smoke, don’t smoke for at least two weeks after surgery. It’s normal to have bruising and swelling after surgery. It will go away with time. Your breasts may feel tender and swollen, but they should feel normal, and look soft and natural again, about three months later. There could be residual spots of numbness for six weeks to six months. A week after breast lift surgery, you can drive yourself and go back to work. The breast is unique to mammals. It differentiates mammals from other species. It serves to provide nourishment from mother to her offspring. The nipple is typically elevated, or everted, above the surface of the breast and contains 15 to 20 milk ducts, which allows for the passage of milk from the breast gland. The nipple is surrounded by the areola, a sheet of fibromuscular tissue and skin. What Causes Inverted Nipples? For reasons we don’t understand, the nipple can become inverted, meaning that it projects into the breast. This usually develops during puberty as the breast grows. Common findings include either shortened ducts, deep fibrous tissue that prevents the nipple from everting, or both. Are Inverted Nipples Dangerous? Aside from preventing a baby from successfully breastfeeding, there are no physical consequences to having inverted nipples. However, inverted nipples are not typically depicted as “beautiful” and as such, many woman who have inverted nipples feel inadequate and very self-conscious. This can cause problems with intimacy and self-confidence. Even many women with inverted nipples, who have stable, healthy relationships, come to the office for correction because they “have always” felt (and continue to feel) unattractive. Levels Of Severity In 1999, a grading system was described by Han and Hong. It is still used today to describe the severity of inverted nipples. In addition to being able to classify a patient’s condition, it helps to predict the success of the variety of different treatments. There are three grades. GRADE I In Grade I, the problem is mild. The nipple can easily be everted with gentle pinching and can usually maintain an everted position for a short time while the areola contracts. Surgical correction of this grade can usually be accomplished with just a buried suture at the base of the nipple. Sensation and the ability to breastfeed should be maintained. GRADE II Grade II inverted nipples can also be manually everted, but it takes a lot more effort. Also, it does not stay everted and quickly becomes inverted when released. Usually the ducts are not too short but being pulled downward by the fibrous tissue. The ducts can be released by dividing the deep fibrous tissue. Adding the buried suture maintains the everted position and completes the procedure. Usually sensation and the ability to breastfeed are maintained, though not guaranteed. GRADE III Grade III is the most severe. The nipple cannot be everted manually. There is a lot of fibrous tissue that keeps the nipple inverted, and the ducts might be physically shortened. This is the most difficult grade to successfully treat. There have been many types of procedures designed to correct these inverted nipples. They all have certain things in common. They divide the fibrous tissue, keep the nipple everted after surgery with a traction suture and add tissue into the nipple. Adding tissue helps to add bulk, which is lacking once the nipple is everted. The tissue usually comes from small “flaps” created from the adjacent areola. Intra-operative view of intact ducts after removal of fibrous tissue Risks Of Surgery In addition to adding bulk, the flaps can serve as a sling to keep the nipple everted. The drawback in these techniques is that there is a lot of cutting beneath and within the nipple. This can divide the ducts, nerves and blood vessels. This can result in the inability to breast feed, numbness and, in some cases, can cause a problem with circulation and healing. Finally, in spite of all of these efforts, there is always a chance of relapse. How Is Surgery Performed? Dr. Schuster’s choice of correction of Grade III inverted nipples is to perform the surgery with the help of a magnifying small joint arthroscope. This technique was first reports by Chen, et al, in 2007. By using the arthroscope, the surgery can be performed through a very small incision placed at the base of the nipple. The scope magnifies the area so that the fibrous tissue can be divided while leaving the ducts, nerves and vessels alone. Then a small strip of skin is taken from a well-concealed area in the groin, and placed into the nipple to provide the needed bulk. A buried suture is added at the bottom and a suspension suture is worn for a week. Although it is a technically more demanding surgery, and takes a bit longer to perform, Dr. Schuster believes it has the highest success rate and the best chance to preserve sensation and the possibility to breastfeed. Is Surgery Painful? The surgery is performed on an outpatient basis under IV sedation. It is not painful after surgery. A slightly bulky bandage is worn for a week. Light protection is needed for six weeks after surgery. Exercise can be resumed three weeks after surgery.

HOW LARGE, HEAVY BREASTS MAY AFFECT YOUR LIFESTYLE AND SELF-ESTEEM Also called reduction mammaplasty, this breast procedure alleviates a wide range of issues associated with heavy breasts: Physical discomfort Chafing and rubbing Back, neck and shoulder aches Redness, irritation or grooves where bra straps dig into your shoulders Difficulty maintaining good posture Discomfort when exercising, running and generally being active Difficulty finding tops and dresses that fit your bust and look flattering Large breasts often run in the family. On the other hand, some women develop these breast characteristics after they’ve been pregnant and breastfeed, or as a result of being overweight. Good candidates for reduction mammaplasty are in good general health and have realistic expectations of what the procedure can achieve. While reduction can address some degree of unevenness and asymmetry between the breasts, it’s normal for some amount to remain. No breasts are exactly identical to one another. A consultation is necessary to determine if this procedure is appropriate. Dr. Schuster will listen to your concerns and perform an exam of the treatment area. For some women, options like weight loss or liposuction may serve as an alternative to breast reduction. More often, they may be utilized as a companion to the procedure. In fact, some women are advised to lose weight before undergoing the procedure so that their breast size can be brought as close to ideal through weight loss before surgical reduction. Weight loss and liposuction do not tighten the skin the way that this procedure does, however. WHAT TO EXPECT FROM BREAST REDUCTION Like all breast procedures, reduction mammaplasty does leave visible scars. Dr. Schuster makes every effort to hide scars in natural creases when possible. There are many theories as to what causes gynecomastia. Abnormal levels of estrogen and testosterone, abnormal hormone receptor activity and/or quantity have all been postulated. In the majority of cases (called idiopathic gynecomastia), there is not any identifiable cause. Idiopathic gynecomastia develops during puberty.
